We pride ourselves in being the premier health care facility in the greater New Haven area!  We have highlighted some of our awards and achievements below, however, we provide award winning care each and every day to enrich the lives of the residents we are entrusted to care for.

• 2014 U.S. News and World report ranked Hamden “number 5” on their list of top nursing homes in Connecticut

• 2010 Hamden achieved Medicare’s 5 Star Rating! Hamden is New Milford’s only 5 star rated skilled nursing facility.

• 2009 – 2010 recipient of My InnerView’s Excellence in Action Award.

SPOTLIGHT ON OUR AWARD WINNING THERAPY TEAM!

Our exceptional therapy team is led by Lisa Fernez, LPTA. They received the Clinical Excellence Team Award from the Preferred Therapy Network for the past five years. This award is based on quality assurance measures as determined by their annual audit. Our team scored 95% on this years audit. This award is also based on the teams clinical strength and mentoring of Quinnipiac and Sacred Heart University Fellowship students. In addition to the Clinical Excellence Team Award, The Preferred Therapy Network also awarded our therapy team the Shining Star Award.

Each therapist is ACP certified. They each had extensive dementia training to stage and treat residents with cognitive impairment. The Hamden team also has an Athletic Trainer on staff!

Our team is the driving force of our Meadowbrook Short Term Rehabilitation unit. We are proud of their commitment and dedication to all the residents on program.
